School Health Team

VULNERABLE CHILDREN & YOUNG PEOPLE

We are a team of specialists and health professionals experienced in working with vulnerable children, young people and their families.

We are from a range of professional backgrounds including general health nursing, school nursing and health visiting.

We work in partnership with universal health services, education, children’s social care services and voluntary agencies to improve health outcomes for vulnerable children and young people (VC & YP) from birth to 19 years of age. This can involve direct work with the individual or an advisory role for other agencies/health care professionals.

We provide training and consultation on vulnerable children and young people’s health issues to help primary care professionals to develop new skills, and knowledge.

We work closely with other professionals to ensure that the health needs of vulnerable children and young people are recognised and assessed early and that they receive the services most appropriate to their health needs.
